      From a bird's-eye view you could say that the speed of building has always outpaced some of the thinking that should have been done :)

      It can be a defect that perfectly working software has, which is worse than a bug.

      This can slip through the cracks or be swept under the rug whether it's intentional or not

      But when the pace is put on steroids, it would be way more difficult for observers to ignore.
