Now that I've looked at the community discussion over on GH and looked at Hashimoto's Vouch system, I'm failing to see how any of the suggested solutions actually place a human-in-the-loop.

I think Vouch does a good job of creating a system of admission, but there's still the effort of reviewing and/or prioritizing which PRs receive attention.
